As required by Freehold Township ordinance, all volunteer coaches and trainers must be background checked every three years. The step-by-step process is as follows:
- Email the following to pkpreston@aol.com and vceravolo@twp.freehold.nj.us:
a. “Freehold Soccer League” (this identifies which organization you are from for Freehold Township Recreation tracking purposes)
b. Name
c. Address
d. Email address
e. Phone #
- Download the Background Check and Fingerprinting Application form.
- Contact MorphoTrak at www.bioapplicant.com/nj or (877) 503-5981 to schedule your appointment for fingerprinting
- You will be notified when your Clearance Letter has been received and then you must contact Vera Ceravolo at Freehold Township Recreation (732) 294-2190 to schedule a time to visit the Freehold Township Recreation Center to have your Photo ID Badge issued
- Please provide a copy of your Photo ID Badge and a copy of your receipt from MorphoTrak to treasurer@freeholdsoccer.com for reimbursement
- You must wear your Photo ID Badge at all times when coaching on Freehold Twp fields
