Two different authorization forms available:

  • Notification of Travel for US Youth Soccer Sanctioned Events

  • Notification of Participation in a non-US Youth Soccer event 

A reminder to all U09 and U10 teams: By policy adopted by the NYSW Membership on 11/18/2001, all U09 and U10 play in tournaments must be advertised/marketed as “Festivals” or “Jamborees.” Format of play must be round robin with no game results recorded/posted. There shall be no awards based on win/loss records for these age divisions—only participation awards for all players. Participation in tournaments that do not meet this criteria is a violation of NYSW policy and could result in your team being subject to penalties.  Travel to events that do not meet this criteria will not be covered by NYSW’s insurance.

 It’s easy to use.

  1. Select your club in the menu list.
  2. Select your team.
  3. Select yourself.
  4. Select which form you need.
  5. Complete the form.
  6. Then use your browser’s print function to print out your copy of the authorization or notification.

  • THESE FORMS ARE INTENDED FOR USE BY NEW YORK WEST MEMBER CLUBS ONLY

    IF YOU ARE TRAVELING TO A US YOUTH SOCCER TOURNAMENT IN REGION 1, IT IS POSSIBLE THAT YOU DO NOT NEED TO DO THIS PAPERWORK. PLEASE READ THE REGION 1 TRAVEL POLICY BEFORE YOU COMPLETE YOUR TRAVEL FORM.

    The Notification of Participation in a non-US Youth Soccer event should be used for those events that DO NOT have a US Youth Soccer Permission to Host.  This includes all US Club Soccer events.  It is your responsibility to know who is sanctioning the event you are attending and to use the correct documentation!

    If you need a tournament guest player form - please go to this form and forward it to the state office for approval.  This is a temporary solution until we get the new electronic guest form built.

    New York State West certified official rosters and member passes are valid for

    US Youth Soccer events only.

    Use the non-US Youth Soccer roster and non-US Youth Soccer passes available in RosterPro when attending events outside of US Youth Soccer.
